The following forms are the papers you will be given when visiting our office.  If you would like you may print needed forms and bring them to your appointment.

Visual Field Screening- 

The visual field screening is a test used to measure a patient’s entire scope of vision as well as their central and peripheral (side) vision.  This test is used to determine damage to the optic nerve and the detection of disease, eyelid conditions and tumors.  This test is $15.00 and not covered by insurance.  (Both Visual Field and Retinal Photography together are $25.00 total.

 Retinal Photography-

Retinal photography assists in the detection and management of problems such as diabetic changes, hypertensive retinopathy, macular degeneration, optic nerve disease, and retinal holes or thinning. 

We recommend that all our patients receive this test. It is especially important for people with a history of high blood pressure, diabetes, retinal diseases, flashing lights, floaters, headaches, or a strong glasses prescription.
